Because their avatar could not be downloaded. Then there is a questionmark used as replacement. - The mails looks weird in Outlook.
Until yet Outlook isn't able to display the mails correctly. Every application based on Safari or Mozilla rendering engine as Mail.app or Thunderbird will display it the right way. But I'm working to fix that issue. - Will I get tweets mentioning my screenname not at the first position?
Yes. Even if your screenname (with a leading @) is mentioned at the middle or end of the tweet you will get these.
TwitteReply is a lightweight service to send you all all messages containing a mention of you nickname including a @-sign. So every reply in definition of Twitter but also messages not counted as reply by twitter but maybe important for you.
